Not sure what is causing it, restarted IIS, deleted my temp internet files etc...  here is the output;

Turn off verbose logging in the Admin -> SMTP section.

ok i did that and it's fine. But is it really a problem? or just something random.   

It was trying to write debug info to that file, seems like the file is / was in use.

When the verbose logging is off it does not write to that file.
