The Issue Type field was blank, and the issue type list is empty. The Status combo is disabled.
I realise that I needed to set up the Issue Type Schemes to correct this problem, but your error checking probably needs to validate and inform the user a bit better of the nature of the problem.
eg. "Please set up your Issue Type Profile for 'Software Development Projects' Also you should not be able to remove an issue type if it was in use in that scheme, rather better to soft delete (mark obsolete)
